Mental Health Consulting
Job Summary
The Mental Health Therapist will work with early childhood programs providing therapeutic services for children while using evidenced based therapy. They will provide clinical services oversight to children experiencing complex behavioral health challenges. Emphasis is on providing a whole-person approach to care and addressing physical and behavioral health.

Summary of Essential Duties and Responsibilities:

Identifies potential health problems through assessment of participants’ physical and mental status, daily living skills, coping patterns, strengths and supports

Deliver medically necessary care coordination services

Use evidence based treatment plans and monitor all documentation for data collection and outcomes

Will provide the data and medical history that will accelerate successful referrals

Ensure annual age appropriate behavioral health screenings using evidence based tools
-
Referral support and bi-directional tracking with communications to admin team
-
Keep accurate documentations of all interventions and determinations of diagnosis summaries
-
Submits referrals to community resources as needed using the program referral procedures

Provide ongoing measurement of individual in service outcomes and evaluation of intervention
